Monaco Gerontological Coordination Centre moves to new premises

Minister of State Pierre Dartout, President of the National Council Stéphane Valeri, Mayor of Monaco Georges Marsan, Minister of Health and Social Affairs Didier Gamerdinger and Minister of Public Works, the Environment and Urban Development Marie-Pierre Gramaglia visited the new premises of the Monaco Gerontological Coordination Centre at 18 Rue Princesse Florestine in La Condamine on Monday.

Medical responsibility for this medical and social care facility under the auspices of the Department of Health Affairs lies with Dr Louchart de la Chapelle, Head of the Geriatric Care Department. The Centre is coordinated by senior healthcare executive Philippe Migliasso, and employs a geriatrician, Dr Pascale Porasso, as well as nurses and social workers. It offers a single point of access within the community, which can be accessed by anyone aged over 60 and living in the Principality who has experienced a loss of physical or cognitive autonomy.

The Centre aims to identify the needs of such individuals through a Standard Geriatric Assessment carried out by a team of medical and social care staff at the person’s home. This assessment is used to draw up an appropriate protocol and a personal support plan.

As part of this work, the facility:

• centralises and facilitates access to information about medical and social care
• manages the geriatric healthcare network (including partners in the public and private health and social care sectors, and healthcare providers working in hospitals and on a private basis)
• assesses and reassesses, on an annual basis, the biopsychosocial and environmental needs of the person within their living environment
• suggests and helps to put in place a personal support plan
• facilitates the return home following discharge from hospital
• reviews the application for an autonomy allowance (financial aid)
• supports family carers by leading support groups
• helps to train medical staff and social workers to care for people with Alzheimer’s disease or related disorders
• disseminates public health messages
• anticipates the need for services and makes future plans based on an analysis of activity
